Applicants must print the complete application and submit to the Main office of Bill Crothers Secondary School, 44 Main Street, Unionville, Ontario, (Application Committee) on or before Thursday December 3rd, 2009, 6 pm. |

All applicants must complete Part 1A, 1B, 1C, and 1D. |

– Part 2A – Print and give to Applicant’s Teacher |

– Part 2B – Print and give to the Coach of the Applicant’s main sport. |

Submission Procedures Form – Print and attach this form to the front of your completed application. Please make sure you have included the complete application and all additional requested
information. (2 report cards (as detailed), IEP (if applicable) and non refundable $30 application fee).
Incomplete applications will not be assessed. |

When you have completed the application, please attach the check list to the front of your application and submit in a sealed 9 x 12 envelope.
